Mental Health Resource Team for Dane County Older Adults

The Mental Health Resources Team (MHRT) supports older adults in Dane County community-based case management programs with mental health needs and helps them access community resources to meet their needs. The team’s goal is to increase collaborations to reduce barriers, support early interventions, and empower older adults with tools that enhance resilience, social connection, and emotional well-being.

Notice of MHRT Program Restructure – 2025–2026

NewBridge is beginning a significant restructuring of the Mental Health Resource Team (MHRT) for the remainder of 2025 and into early 2026 due to a significant budget reduction. During this transition, we will continue serving individuals already referred and those currently engaged in services. However, we will pause new referrals as we adjust to the budget reductions and redesign our service delivery model.

MHRT staff will prioritize consultation and support for Case Managers to help maintain continuity of care during this period. Thank you for your understanding and collaboration as we move through this transition together.
